My Nurses Registry can help you transition smoothly from the hospital to your home, including care coordination and medication monitoring by a licensed nurse.
To help with the patient’s transition home, we can arrange transportation, equipment delivery, and prescription pick-up. Before the patient returns home, you can schedule a pre-discharge home visit with us to give a home safety evaluation and establish the patient’s needs beforehand.
